Birde bu \u00fc\u00e7 tabakay\u0131 birbirinden ay\u0131ran iki elastik yap\u0131 var (Lamina elastika eksterna ve interna). Klasik olarak eskiden \u015fu biliniyordu; damar sertli\u011fi yapan lipitler, oksi LDL ve 250 den fazla etkenler damara \u0130ntimal tabakadan,&nbsp; yani damar\u0131n en i\u00e7 \u00f6rt\u00fcs\u00fc olan ve tek tek h\u00fccrelerin s\u0131ralanmas\u0131ndan olu\u015fan Endotel&#8217;den girer ve \u0130ntimal tabakaya oturur ve burada birikirler, Lipid plaklar\u0131n\u0131 olu\u015ftururlar. Bu lipid (ya\u011f) olu\u015fumlar\u0131, \u00f6nce biraz damar\u0131n d\u0131\u015f\u0131na do\u011fru b\u00fcy\u00fcme g\u00f6sterse de (remodeling=yeniden \u015fekillenme), sonra damar\u0131n i\u00e7ine do\u011fru b\u00fcy\u00fcme yapar. B\u00f6ylece plaklar olu\u015farak damar\u0131n i\u00e7i daral\u0131r. Bu plaklar\u0131n \u00fczeri bir tabaka ile kapl\u0131 olup baz\u0131 plaklarda incedir ve&nbsp; bunlara hassas (Vulnerable) plak denir. Plak \u00e7o\u011fu zamamn&nbsp; omuz k\u0131sm\u0131&nbsp; denilen b\u00f6lgeden, ya da \u00fcst\u00fcn\u00fc kaplayan kaps\u00fcl\u00fcn en ince yerinden \u00e7atlayarak i\u00e7indeki lipitkoru ve komponenti damar\u0131n i\u00e7ine do\u011fru bo\u015fal\u0131r. Bu esnada koruyucu fakt\u00f6rler, ba\u015fta trombusler gelir bunun etraf\u0131nda toplan\u0131r ve burada t\u0131ka\u00e7 olu\u015fturur. Neticede damarda i\u00e7ten t\u0131kan\u0131kl\u0131k veya darl\u0131k meydana gelir. Olu\u015fan damar t\u0131kan\u0131kl\u0131\u011f\u0131 bazen kan ak\u0131m\u0131n\u0131n tam durmas\u0131na sebep olur, bazen de k\u0131smen gitmesini engeller. T\u0131pta bu duruma \u201cdamar\u0131n i\u00e7ten t\u0131kanmas\u0131\u201d deniyor. Damar sertli\u011finin, Media tabakas\u0131nda olu\u015ftu\u011fu zaten daha evvelce ispatl\u0131yd\u0131. Biz ise; damar\u0131n her tabakas\u0131nda bu i\u015fin olabilece\u011fini ortaya \u00e7\u0131kard\u0131k. \u00c7al\u0131\u015fma Grubumuz ile yapt\u0131\u011f\u0131m\u0131z incelemelerde; koroner arter aterosklerozunda plak olu\u015fumu ve kalsifik tutulum, klasik intimal kaskad yolu d\u0131\u015f\u0131nda yani yaln\u0131z endotel h\u00fccreleri alt\u0131ndan de\u011fil, adventisya tabakas\u0131ndan da ba\u015flayabilece\u011fini ve ilerleyebilece\u011fini ve damar\u0131n l\u00fcmeninde (i\u00e7inde) daralmaya, t\u0131kanmaya sebep olabilece\u011fini ortaya koyduk.\u201d<\/p>\n\n\n\n<p>\u0130LA\u00c7 TEDAV\u0130S\u0130 \u00d6L\u00dcMDEN KURTARACAK<\/p>\n\n\n\n<p>Damar t\u0131kan\u0131kl\u0131\u011f\u0131n\u0131n sadece endotel h\u00fccreleri alt\u0131nda (\u0130ntima) veya M\u00f6nckeberg\u2019in tariff etti\u011fi arterin orta tabakas\u0131 olan Muskuler Media tabakas\u0131nda olabildi\u011fi gibi, atardamar\u0131n (arterin) en d\u0131\u015f tabakas\u0131 olan Adventisya dan da ba\u015flayabilece\u011fini d\u00fcnyada ilk kez tespit etmekle kalmad\u0131klar\u0131n\u0131 belirten Prof. Dr. Erzengin, damar t\u0131kan\u0131kl\u0131\u011f\u0131yla ilgili d\u00fcnyada ilk olan te\u015fhisin yan\u0131nda, devaml\u0131 ilerleyen bu hastal\u011fa ekibiyle birlikte ila\u00e7la \u00e7\u00f6z\u00fcm ve tedavi y\u00f6ntemleri de geli\u015ftirdiklerini ifade ediyor ve \u015f\u00f6yle diyor: \u201cGayemiz sadece damar t\u0131kan\u0131kl\u0131\u011f\u0131yla ilgili yeni te\u015fhisler ortaya koymak de\u011fildir.<\/p>\n\n\n\n<p>Bu hastal\u0131k, y\u0131llard\u0131r durdurulamaz ve hatta geriletilemez olarak bilinirdi, biz bu hastal\u0131\u011fa kar\u015f\u0131 y\u0131llard\u0131r yaln\u0131z ila\u00e7la \u00e7\u00f6z\u00fcm ve medikal tedavi y\u00f6ntemleri \u00fczerinde de \u00e7al\u0131\u015ft\u0131k ve \u00e7ok belirgin ba\u015far\u0131lar elde ettik. \u00d6zetle aterosklerozun (dammar sertli\u011finin) durdurulabildi\u011fini ve tamamen geriletebildi\u011fini ortaya koyduk. Uzun zamand\u0131r DETEA ve benzeri ileri tetkik ve deney laboratuvarlar\u0131m\u0131zda atelostolozun tiperini, sebeplerini, olu\u015f nedenlerini ve olu\u015f mekanizmalar\u0131n\u0131 ara\u015ft\u0131rd\u0131k.<\/p>\n\n\n\n<p>Hastal\u0131\u011f\u0131n y\u00fczde 60 kadar\u0131 klasik intima yoluyla ger\u00e7ekle\u015fiyor ama M\u00f6nkemek\u2019in tarif etti\u011fi skreloz y\u00fczde 3 kadar bir oranda damar sertli\u011fini olu\u015fturuyor. Bize g\u00f6re dammar sertli\u011finin (aterosklerozun) geri kalan y\u00fczde 35-40 kadar\u0131 Adventisyadan ba\u015fl\u0131yor. Bu da ya kalbin etraf\u0131ndaki epikardial ya\u011f tabakas\u0131ndan kom\u015fuluk yoluyla (diffuzyon yoluyla) veya atardamarlar\u0131 besleyen arterioller, yani incecik k\u0131lcal arterlarle (vasevasorum denilen, atar damar\u0131n kendisini besleyen temiz kan\u0131 getiren k\u0131lcal atar damarlar yoluyla)&nbsp; tekrar belirteyim klasik kaskaddaki endotelden giren 250 den fazla fakt\u00f6rlerin,bu ikinci de\u011fi\u015fik bir yol olan arterin d\u0131\u015f tabakas\u0131na ula\u015fmas\u0131yla olu\u015fuyor.<\/p>\n\n\n\n<p>NOBEL\u2019E ADAY OLAB\u0130LECEK B\u0130R \u00c7ALI\u015eMA<\/p>\n\n\n\n<p>Bilimsel \u00e7al\u0131\u015fmalar\u0131ndaki hedefi; \u201cdamar sertli\u011fini \u00f6nlemek ve bu hastal\u0131k nedeniyle daha \u00f6nce by pas amaliyet\u0131 oldu\u011fu halde, hastal\u0131\u011f\u0131 devam edenlerin rahats\u0131zl\u0131\u011f\u0131n\u0131 tamamen ortadan kald\u0131rmak\u201d \u015feklinde \u00f6zetleyen Prof. Dr. Erzengin, \u00e7arp\u0131c\u0131 sonu\u00e7lar\u0131 olan \u00e7al\u0131\u015fmalar\u0131na dair g\u00f6r\u00fc\u015flerini \u015fu s\u00f6zlerle anlat\u0131yor: \u201cS\u00fcrekli ilerleyen bir process olan ve ciddi \u00f6ld\u00fcr\u00fcc\u00fc sonu\u00e7lara yol a\u00e7an bu arterleri tutan damar sertli\u011finin, yaln\u0131z ila\u00e7lar yoluyla tedavisini bulmay\u0131 ama\u00e7lad\u0131k.
